SCREENING FORM
For faster screening, be sure to fill the form out completely. The email you put on this form is where I will contact you initially. If you do not have references, you can leave that part blank. You will only need to screen once. In the future, you can simply email or text me to make a booking. Please note that I require a 25% deposit for all new clients, travel bookings, and dates longer than 5 hours.
